Should i undergo radioactive iodine treatment or not?
I've had hypothyroidism for a year then hyper, in the end I had a goitre and 2 malignant nodules in my thyroid (found out in early march). The big one was 22 mm (partially capsular, shows focal capsular invasion) and the small one 1.5 mm. (no capsule), no metastasis. So the doctor said I don't need the iodine treatment, because of my age and size of tumor and it could be back with or without treatment if it does (he went into the operation theatre); the surgeon said the same-I don't need it. Later on I met 2 other docs (endocrine) asking about it and the 1st thing they asked whether I had done the treatment already and that I have to do it. The operation was in late march, 2 months ago, and I don't know what to do. I don't want to have cancer back after 20 or 40 years. I want to be free of it. My thymoglobulin at that time was 16.6 ng/ml, now after thyroidectomy it is 1.1 ng/ml (I know it has to be less than 1) now antithyroglobuline is less than 20.0 ul/ml I'm 28. Papillar carcinoma. No neoplasia in surgical margins. No adenomas away from main tumor. No evidence of vascular invasion. No evidence of extra thyroid extension. Rest of thyroid tissue away from tumor: inflammatory infiltrate focal lymphocytic type. I have no problem in undergoing the iodine treatment but don't know if i need it or not. Thank you doctor. Thyroid Advice About Radioiodine TherapyThis is difficult to answer as you have conflicting advice between surgeons and physicians (endocrinologists). Any advice I give is likely to compound the problem, but I would fall on the side of the physicians or endocrinologists here. We know that in total thyroidectomy, it is rare for all the thyroid tissue to be removed at surgery so it is possible for cancer tissue or cancer potential cells to be left behind. The radioactive iodine therapy will help to destroy any of those cells left behind so I would suggest following the endocrinologists advice in this matter.
