Passing Faeces In My Urine

What could be the cause of passing out watery faeces through the penis by a patient with cancer of the rectum?


Answer:

Passing watery faeces in the urine is due to a fistula or abnormal connection between the bowel and the bladder. This is also known as an enterovesical fistula and can be thought of as a tunnel between two organs. Fistulae can occur anywhere where there are two organs close by. It can be caused as a direct affect of the rectal cancer invading the bladder wall causing a fistula tract to occur. It can also occur due to localised radiotherapy with resultant inflammation causing the two surfaces to “stick together” and result in fistula forming.

Other conditions such as diverticular disease can also cause an enterovesical fistula to form through infection and inflammation occurring. Surgery to the area can also cause this through adhesions and localised inflammation.

If due to the cancer, which seems the most likely cause, localised radiotherapy treatment and surgery may be helpful in this situation. It is a significant problem as it can cause infection of the bladder or cystitis, which results in painful urination, frequency of urination, incontinence and fever which can lead on to complications such as pyelonephritis and septicaemia.

If experiencing these symptoms you should seek urgent advice from your oncologist. You may need treatment with antibiotics to help prevent infection from the bacteria contained within the faeces as well as treatment for the fistula itself. Your oncologist will be able to advise you on the best course of action for your enterovesical fistula as well as investigating it further or referring you to a surgeon for surgical correction of the fistula. Drink plenty of fluids to keep your kidneys flushed through and to prevent dehydration. Don’t delay, get this checked out as soon as possible.
