“I’m a 32 year old woman and have rectal bleeding. It is bright red and with clots. It is not in the stool but there is bleeding when I pass gas or open my bowels. What could be causing this to happen?”

Causes of rectal bleeding that’s bright red with clots and coats the stool suggests lower GI bleeding. Usually this occurs in the left colon for it to be bright red with clots, although severe bleeding can come from further up the colon or even from the small intestine in some cases.

In a lady of your age the most likely cause is haemorrhoids. These blood vessels at the level of the anus or “back passage” occur usually due to excessive straining and are commonly seen in sufferers of constipation. However, they can occur in anyone and are extremely common.

The fact you have also passed clots could suggest other rectal bleeding causes such as a problem in the rectum or higher up. This might include diverticular bleeding, abnormal blood vessels in the bowel wall (angiodysplasia) or a polyp or even a tumour (although rare in a 32 year old unless there is a family history of colon cancer <45 years old) and in my practice I would normally advise a flexible sigmoidoscopy or even a colonoscopy to check this.

Treatment really depends on the rectal bleeding cause. Haemorrhoids are usually easy to treat with creams, suppositories and surgical correction. Polyps can be removed endoscopically normally. Diverticular bleeding usually settles spontaneously, although sometimes requires hospital management.
