abnormal bleeding between periods.

(united kingdom)
I’m 40 years old and I’ve been bleeding between periods since 30 of December 2010, I went to my doctor’s nurse and she gave me a smear and test for Chlamydia, both came back ok. I have in the past had abnormal results from smear test which I sought treatment for. The bleeding can be like a tap running and this can happen frequently through the day and also it can be like spotting. Sometimes I’m in excruciating pain. The nurse did prescribe me Norethisterone 5mg which I have been taking for four days so far and this still hasn’t worked. Deep down I just feel like there is something there missing and cant stop worrying about this. Please I would be more than grateful if you could try to answer my question.
Abnormal Vaginal BleedingSorry to hear of your abnormal vaginal bleeding. Clearly, this has been causing you a lot of grief and worry which I can fully understand. However, it is usually idiopathic or of unknown cause. You have had a smear test which was clear, but It might be worth asking your doctor about having an ultrasound to make sure that there isn’t any structural abnormality of the uterus such as fibroids causing your symptoms. Endometriosis could also underlie your problems. Treatment of Abnormal Vaginal BleedingTreatment of abnormal vaginal bleeding has several approaches. You have been tried on Norethisterone, a hormonal manipulator to help ease your symptoms. I note it hasn’t worked so far, but it has only been 4 days and you may need to see how things are after 1-1/2 month’s treatment. Other options you could consider are using a Mirena coil to help control your symptoms, endometrial laser ablation therapy and even considering hysterectomy or removal of the uterus. These options would only be realistic if you no longer wish to become pregnant. My name is Dr Peter Thatcher MBBS, MRCPI and I am a consultant physician and gastroenterologist in the UK.
